Detection of intramural scar by electroanatomic mapping versus MRI in patients with non-ischemic cardiomyopathy

چکیده
Methods In 15 consecutive patients with structural heart disease and without contraindication to MRI, intramural scar was detected by DE-MRI (Fig 1a). All patients underwent endocardial voltage mapping by catheter. Both unipolar and bipolar voltage maps were constructed from the endocardium. The scar on DE-MRI was registered with the electroanatomic map using fiducials (apex, aorta, mitral valve) and a polar representation of the corresponding endocardial voltage was generated (Fig 1b: red points: low voltage, purple points: high voltage, yellow and green points: fiducials).

Delayed-enhanced magnetic resonance imaging in nonischemic cardiomyopathy: utility for identifying the ventricular arrhythmia substrate.
OBJECTIVES The purpose of this study was to assess the value of delayed-enhanced magnetic resonance imaging (DE-MRI) to guide ablation of ventricular arrhythmias in patients with nonischemic cardiomyopathy (NIC). BACKGROUND In patients with NIC, ventricular arrhythmias often are associated with scar tissue. DE-MRI can be used to precisely define scar tissue. METHODS DE-MRI was performed in ...
متن کاملCharacteristics of intramural scar in patients with nonischemic cardiomyopathy and relation to intramural ventricular arrhythmias.
BACKGROUND Ventricular arrhythmias have been described to originate from intramural locations. Intramural scar can be assessed by delayed-enhanced MRI, but MRIs cannot be performed on every patient. The objective of this study was to assess the value of voltage mapping to detect MRI-defined intramural scar and to correlate the scar with ventricular arrhythmias. METHODS AND RESULTS In 15 conse...
متن کاملImpact of nonischemic scar features on local ventricular electrograms and scar-related ventricular tachycardia circuits in patients with nonischemic cardiomyopathy.
BACKGROUND The association of local electrogram features with scar morphology and distribution in nonischemic cardiomyopathy has not been investigated. We aimed to quantify the association of scar on late gadolinium-enhanced cardiac magnetic resonance with local electrograms and ventricular tachycardia circuit sites in patients with nonischemic cardiomyopathy. METHODS AND RESULTS Fifteen pati...
متن کاملEpicardial substrate mapping for ventricular tachycardia ablation in patients with non-ischaemic cardiomyopathy: a new algorithm to differentiate between scar and viable myocardium developed by simultaneous integration of computed tomography and contrast-enhanced magnetic resonance imaging.
AIMS During epicardial electroanatomical mapping (EAM), it is difficult to differentiate between fibrosis and fat, as both exhibit attenuated bipolar voltage (BV). The purpose of this study was to assess whether unipolar voltage (UV), BV, and electrogram characteristics (EC) can distinguish fibrosis from viable myocardium and fat during epicardial EAM for ventricular tachycardia (VT) ablation i...
متن کاملCharacterization of trans-septal activation during septal pacing: criteria for identification of intramural ventricular tachycardia substrate in nonischemic cardiomyopathy.
BACKGROUND Identification of intramural basal-septal ventricular tachycardia (VT) substrate is challenging in nonischemic cardiomyopathy. We sought to (1) characterize normal/abnormal trans-septal right ventricular (RV) to left ventricular activation; (2) assess the effect of opposite RV pacing on left ventricular septal bipolar electrograms (EGMs); and (3) establish criteria for the identifica...
